IVF and ICSI are not competing treatments — they are two fertilization methods inside the same treatment cycle. Ovarian stimulation, monitoring, egg retrieval, embryo culture, and transfer are identical. The fork in the road is a single step in the laboratory:
Conventional IVF insemination: tens of thousands of prepared sperm are placed with each egg in a culture dish, and fertilization happens on its own. ICSI: an embryologist selects one sperm and injects it directly into each mature egg under a microscope. That's the entire difference.
For couples with normal sperm parameters, large randomized and registry studies show that ICSI does not improve fertilization proportion, pregnancy rates, or live birth rates compared with conventional insemination. Its one genuine advantage in that group is insurance against the small (roughly 2–5%) risk of unexpected total fertilization failure.
Worldwide, ICSI is now used in around two-thirds of all cycles — far beyond the share of couples with a true male-factor indication. Many clinics default to it for predictability. Only when there's a reason for it. With male-factor infertility, ICSI dramatically improves fertilization. With normal sperm, high-quality evidence shows no improvement in pregnancy or live-birth rates over conventional IVF insemination — its main benefit there is avoiding rare total fertilization failure.

Yes — a split insemination cycle fertilizes part of the eggs each way. It's occasionally useful in first cycles with borderline sperm or unexplained infertility, and it answers the question for future cycles definitively.
It happens in roughly 2–5% of conventional cycles. The following cycle is then done with ICSI, which resolves the problem in the great majority of cases. This risk is the main reason some couples without male factor still reasonably choose ICSI upfront.
